ADDITIONAL COVID-19 CLEARANCE REQUIREMENTS
Please fill out the webform below as soon as possible if you wish to participate in intercollegiate sports during the 2020-21 academic year. Preferably, prior to arriving on campus.

You must contact Student Health Services at 860-439-2275 for further instructions BEFORE COMING to campus if you are CURRENTLY experiencing any symptoms of COVID-19.
If you are identified as being at risk for severe illness from COVID-19 infection you will receive an additional form that you and your primary care provider must sign to clear you for participation in intercollegiate athletics.
Additional preparticipation medical clearance measures, such as cardiovascular and respiratory exams, will be required prior to participation in athletics for any student-athlete that has had COVID-19.
